Our office staff are ready to help you with any questions you may have about scheduling, insurance and available financing options, to make this part of the process as simple as possible.

Administrative information about our practice;

Your First Visit

Your initial appointment will consist of a comprehensive examination. The exam is the critical information gathering that will enable us to develop the very best options for you to consider. Depending on the complexity of your case the exam may include;

  • Photographs – Models of your teeth
  • Medical Review – X-Rays
  • Smile Analysis – Your Goals Desired

Dr. Nelson will explain your diagnosis and your treatment plan, which will have several options for your consideration.

Please provide the following information at your consultation;

  • A complete list of over the counter and prescription medications you are taking.
  • Your referral slip and any X-Rays

You may request that your previous dentist forward X-Rays to our office via mail. If the X-Rays are in a digital format they may be forwarded to

  • Insurance card

A parent of guardian must accompany all minor patients under the age of 18 at the consultation appointment.


Dr. Nelson is open Monday through Thursday 7:30am to 4:30pm and every other Friday from 7:30am to 2:00pm.

If you have an emergency situation please contact us and we will do our best to see you on the same day. We understand your time is valuable and we pride ourselves on staying on schedule and minimizing wait times.

Nitrous Oxide

Many people experience anxiety during a dental treatment. We provide several options to help patient relax. Nitrous Oxide, also known as laughing gas is one of those options. Nitrous Oxide has proven extremely successful in reducing apprehension and helping calm your nerves. The quick onset and reversibility of Nitrous Oxide allows you to drive yourself to and from your appointment while still experiencing a tremendous anxiety reduction during your appointment.

If you need to be more heavily sedated, Dr. Nelson may suggest oral conscious sedation, which involves administering anti-anxiety medication before treatment. During treatment, we will closely monitor your vitals and comfort throughout the entire procedure. Many patients remember very little about their procedure. If you chose oral conscious sedation, you will need a ride to and from your appointment.

Insurance Information

Dr. Nelson makes every effort to provide you with excellent care and convenient financial options. Our office manager and administrative staff will work with you to maximize your insurance benefits or reimbursements for all covered procedures.

Please bring your insurance card or information with you to your consultation so we assist you with insurance billing or reimbursement.

Patient Billing and Financing Options

For your convenience we accept Visa, Mastercard and CareCredit

To deliver excellent care at the most reasonable cost, we require payment at the time of service unless other arrangements have been made in advance.

Our office will send you a monthly statement. The majority of insurance companies will respond within three to six weeks. Any remaining balance after your insurance company has paid is your responsibility. Your prompt remittance is very much appreciated.
